Remedy Recruitment Group are looking for a Personal Advisor for their Leaving Care Team.

Job Purpose

The Medway Leaving Care Team works to advise, support and befriend young people from the age of 16 who are in care and those young people who have recently left care (up to the age of 25) with a diverse range of complex needs to enable them manage the transition into early adulthood .

The PA will provide and coordinate services for young people leaving care and to identify their specific needs, those at risk of harm through assessments, formulation of and statutory review of their pathway plans

The PA will manage a caseload in compliance with departmental policies and procedures within statutory timescales, whilst maintaining accurate records and case files. The PA will manage an agreed caseload of young people (aged 16-25) in compliance with casework objectives, departmental policy and procedural

requirements, consistently maintaining the highest level of professional standards. The PA use agreed assessment frameworks in collaboration with the young person to robustly assess and review their needs, identify risks and contingency plans and to develop corresponding Pathway Plans setting out clear goals

You will need to be:

HCPC registered
