Job description
Our client Bolton Council are looking for a QSW to join their Integrated Discharge team.
Monday to Friday
8.45 to 5pm
No overtime allowed for agency workers
We need people that have had hospital social work experience
Bolton use Liquid Logic so a knowledge of this system would be helpful
They need to work with the team in the office & also on the hospital wards- this is not a home working post
There responsibilities are too;
Hold a caseload of patients that need a Care Act assessment to facilitate safe discharges from hospital
Think home first - with packages of care to meet needs and assistive technology
Working closely with all MDT members in the hospital
To complete Mental Capacity Assessments and make best interest decisions under the Mental Capacity Act
To support Community Assessments Officers
To Attend a daily MDT and report back to the allocated worker actions from the MDT
To provide daily updates to IDT managers for the Criteria to Reside report
To work flexible and pick up urgent pieces of work when needing
Prioritising your caseload daily to meet the demands of patient flow
To be able to work at a fast pace
To be able to recognise Safeguarding concerns and report to managers for support and guidance
Interview with be via Teams with with 4 questions and a case study to disuss the case study is attached and can be given to any candidates that we invite to interview
Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
