• The Community Hospital Social work team are an adult social work team currently based in the community working to support discharge planning for adults in Stockport.
• We work closely with our heath partners to provide support and care under the discharge to assess model.
• We are looking for an assistant team manager who has experience working with adults, and hospital discharges preferably
