Job description

Full time social worker required to fulfil the role of discharging individuals from the acute site, hospice, community beds and Intermediate care. The role will be within the better@home team and comply with the discharge to assess model to facilitate safe and timely discharges. The Better@home team is a multi disciplinary team inclusive of nurses, therapies and social care staff. The team adopts a home first philosophy whilst undertaking the discharge to assess model, working alongside a reablement team.

The candidate must be flexible, a car driver and able to work weekends on a rota basis. The candidate will be assessing individuals within 24/48hrs of discharge either in their own homes or bed based facility. Candidates must be able to work 37 hrs flexibly with occasionally working up to 6pm should the need arise.

You will be an enthusiastic and committed practitioner, capable of building professional relationships with adult service users their carer(s) and families. You will be an excellent communicator who understands the value and importance of integrated working with partner agencies and the community. You'll be efficient in time management, report writing and completing assessments and a strong team player who is used to working in a person-centred way. You will have a holistic approach to work with a keen focus on reducing inequalities and supporting individuals and/or their carer(s)/families to develop their resilience and improve their outcomes. You'll have a positive outlook with a caring and compassionate disposition, be a good listener and a great problem solver.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
