Job description

As a Social Care Officer in the Community Adult Health Services (CAHS) Team, you will work with social workers and other healthcare professionals in completing assessments, arranging and reviewing support in a strengths-based approach so as to, provide a timely, joined-up service that would enable holistic community support, promote wellbeing and independence whilst working in partnership with families and carers.

Referral pathways for CAHS Team Services:
The service is open to anyone aged 18 years and over living within the borough of Wandsworth, registered with a Wandsworth GP and the main referral pathways are as follows:

1) GP referred patients - review of these patients' social care needs, completing statutory assessments and arranging services to assist and support the most vulnerable patients to avoid unnecessary admissions due to social care breakdown.

2) CAHS Multidisciplinary Team Meetings - attend and contribute to the discussion of the patients on the caseload including PACT Enhanced Care Pathway (ECP) patients and identify patients that would benefit from a social care assessment.

3) Quick Start Home Care - working closely with the Quick Start Home Care service by completing social care assessments in a timely way, ensuring that patients do not exceed the seven day provision if they have ongoing needs.
