Waltham Forest Council are looking for a Transforming Care Manager to join there Integrated Commissioning Team.

This is an exciting and key role in the Integrated Commissioning Team. You will support the Integrated Commissioning Manager for Learning Disabilities to ensure health and social care commissioning aims and objectives are achieved for residents of Waltham Forest with learning disabilities and autism. Our primary aim is to reduce the inequalities experienced by and improve outcomes for people with learning disabilities and autistic people in Waltham Forest through driving improvement across health and social care aligned to the NHS Long Term Plan and Waltham Forest Autism Strategy. You shall lead and support, under the direction of the Integrated Commissioning Manager for Learning Disabilities, commissioning initiatives, activity, programmes, and projects which are aligned to achieving our aim.

You will support the Integrated Commissioning Team in driving transformation and achieving value for money in planning, commissioning, and service development for residents of Waltham Forest with a learning disability and autism. You will primarily implement, maintain and lead quality assurance processes for Waltham Forest residents who form part of the 'transforming care' service user group, who are some of our most complex residents with needs arising from learning disabilities, autism, mental health conditions and/or other complex needs. You will be responsible for quality assurance, monitoring and oversight of the care, treatment and support of individuals who have learning disabilities and autistic people who are at risk of admission in the community or are currently admitted to hospital. You will maintain or improve current admission avoidance processes and protocols, facilitate discharge from hospital to suitable community care and support (including for long stay patients who have been in hospital 5year+) and monitor and quality assure the care, treatment and support provided to this client group in in-patient and community settings.

As this is an integrated role between health and social care/ CCG and Council the postholder will need to develop close working relationships with key stakeholders across health and social care in Waltham Forest, primarily within the Community Learning Disability Team, mental health wards and community provider services.

The key priorities for the transforming care manager will be to maintain and improve admission avoidance processes and protocols in Waltham Forest, to proactively facilitate the discharge of people with learning disabilities and autism from hospital- including long stay (5+ years) patients, to support achievement of NHSE targets and trajectories (including reporting on these areas) and providing support to work programmes which enable achieving our target of reducing inequalities experienced by this group.
