Job description

Our client South Gloucestershire Council is looking for a Adult Mental Health Practitioner to join their team.

Reporting to the Team Manager within Avon and Wiltshire Partnership (AWP) Trust Integrated Mental Health services (Thornbury), you will assess, plan and where appropriate provide and evaluate services to people who have, or are the carers of individuals who have, serious mental health needs which fall within the current shared eligibility criteria of the Local Authority and AWP Trust.

You will work with other agencies and services within an integrated Health and Social Care model. Operating within the integrated mental health service, always under the overarching framework of CPA, you will act as care co-ordinator for service users, ensuring the co-ordination and implementation of support which promotes independence in a way that meet the requirements of the Mental Health Act, Care Act, Mental Capacity Act, departmental policy and other locally agreed procedures.

We are looking for one SW however, we may be able to also offer some part-time hours for a second candidate.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
