The team has cases allocated from the court teams following the conclusion of care proceedings and holds case responsibility until a child/young person is no longer looked after or they move to the Young Adults service.
I require workers with a high level of experience in working with looked after children and young people and an ability to care plan. Working with children, young people, their families and carers as well as collaboratively with other professionals in a must. Experience of building and maintaining relationships with children and young people needed. Good knowledge of contextual safeguarding is needed. Key tasks are care planning, direct work, undertaking LAC reviews, child and family assessments. The additional responsibilities will be visiting children and young people, attending PEPS, court and other key appointments for young people.
