Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within Wandsworth Councils Hospital Team at St Georges Hospital.

Job Purpose

• Social worker required to work within Discharge to Assess Service, within an Multi-Disciplinary Team working alongside OT's, Physiotherapists and acute staff.
• Experience of hospital social worker preferred but not essential. Candidate will be required to work on site at St Georges Hospital or Battersea Studios. Home working also available.
• Candidate required to have excellent knowledge of The Care Act, Mental Capacity Act, DoLs and Safeguarding. Candidate will need to be highly motivated, use initiative and manage complex case load. The hospital team is a fast paced and candidate will need to be able to respond and adapt quickly to risk to ensure that appropriate discharge decisions are made.
• Discharge to Assess process means that most face to face assessment will now take place in the community and not in hospital.

You will need to be:
