Surrey County Council are looking for an experienced Social Worker to join their hospital Discharge team, which is based at St Peters Hospital in Chertsey. They are looking to recruit a QSW who is HCPC registered and has completed their ASYE if qualified post 2012.

This role will see you engaging with individuals, families and groups, to assess and intervene using professional judgement to employ a range of interventions promoting choice, control and independence.
