Qualified Social Worker - Somerset County Council - Hospital Discharge Team - Up to £33ph!

Social Worker required to support discharge team within Sedgemoor and West Somerset. The Social Worker will be linked mainly to Minehead community hospital which is a 19 bed unit.

This will be to work alongside another social worker supporting discharges.

They will be expected to carry out timely assessments, gain funding authorisation to support care planning and facilitate discharge.

They will be expected to attend two weekly PDF meetings at the hospital to discuss all patients within the hospital and represent ASC. Some work will be complex and require MCA, BI decisions including at times court of protection work.

They will need to be confident with systems as there will be an expectation to record on both ASC and Health systems. The role is for six months initially. Ideally they will be located in the area and readily available to attend the hospital as and when needed outside the PDF times.
