Qualified Social Worekr - Brent Council - Community Learning Disability Service - Up to £32ph!

The Brent Community Learning Disability Service is a forward thinking team which is co-located within the Kingswood Centre. The Brent Community Learning Disability Service is part of the Learning Disability Service at Kingswood Centre, which has been rated Outstanding by CQC. The team works closely with residents, carers and Independent Providers to provide a person centred service to adults with Learning Disabilities.

We are looking for an experienced practitioner who will hold a varied caseload, which they will manage. You will deliver high-quality care to the wider community. This will mean producing high-quality work in terms of the Care Act and associated legislation, assisting with challenging cases in terms of the CTR process, and the ability to respond promptly to crises as well as hospital discharges.
Reporting to the Team Manager you will help reshape the way we deliver services to our residents in a COVID world. This will involve working to improve the life chances of those with LD/Autism through employment, accommodation, meaningful activities, as well as person-centred services, utilising best practice. You will have excellent communication skills. This will include producing high-quality support plans and working with families and networks against a backdrop of competing demands.
