Qualiifed Social Worker - Nottingham City Council - Adult Locality Team - Up to £30ph!

• Majority of work will be remote based, with occasional visits as required.
• Duties involve: Person-centred assessments, to empower individuals to make choices that promote their health, well-being and independence.
• Collaboratively working with people who have complex needs to formulate support plans that maximise abilities, enable individuals to make choices and to work towards their identified goals.
• Providing an integrated approach to assessment and providing care with key partners.
