Our client, North Lincolnshire Council is looking for a Social Worker to join their Hospital team.

Experienced Adult Social Care worker required to work in the front line service within the Hospital Social Work Team to facilitate same day discharges under the Discharge to Assess model and undertake face to face Care Needs Assessments with people in their homes or short stay Care Home placements within the community.

Previous experience of working within a Hospital Discharge Team or Duty Team is preferred due to the fast paced nature of the role however all applicants with relevant knowledge, experience and application of legislation relating to Adults will be considered.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
* Substantial experience in a Referral & Assessment team.
