Our client, Derbyshire County Council is looking for a Social Worker to join their Hospital Discharge team.

Although the office base is Glossop The Hospital Discharge Team work across the High Peak and are currently working from Home.

Working across Stepping Hill Hospital (Stockport NHS Foundation Trust), Tameside General Hospital (Tameside and Glossop Integrated Care NHS Foundation Trust), Macclesfield Hospitals (East Cheshire NHS Foundation Trust) and local community hospitals in the High Peak.
The post holder will be expected to work Monday - Friday discharging Derbyshire residents via the Discharge to assess model via the appropriate pathway.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
