Job description

Our client Cornwall Council are looking for a qualified Social Worker to join their Discharge team. Working in a multidiscipline team to prioritise hospital discharges under time constraints.

Full time working Friday - Monday inclusive, with hours to meet the demands of the service.

Under the leadership and management of a Team Manager or Service Manager, the Principal Social Worker (PSW) provides a statutory social work service, particularly in the most complex of cases, to adults with health and social care needs achieving high quality, timely and person-centred needs assessment, analysis, care planning, intervention and review appropriate to the work of the team or service unit. The PSW plays a key role in developing, supporting and monitoring the capabilities of front line social care practitioners through modelling and championing expertise in specific areas of social work practice, driving excellent practice based on research evidence and professional experience and supporting the achievement of improved outcomes for adults aimed at enabling self-determination and independence. To provide supervision, mentoring and support to less experienced colleagues including practice teaching for trainee/student social workers. Provide advice and consultation to colleagues from other agencies. The PSW deputises for the Team Manager/Service Manager and represents the team in Task and Finish Groups set up to raise the quality of practice and improve the effectiveness of the service, working closely with the Principal Adult Social Worker and Practice Educators to build learning teams and to redesign adult social work in Cornwall

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
