Job description
Our client Bradford Metropolitan District Council is looking for a qualified Social Worker to join their CMHT. This is a care coordination post in a busy adult CMHT. You will be seeing people face to face and working from an office base, you will be devising recovery focused care plans and completing care act assessments, you will need a working knowledge of system 1 and you will be expected to manage a complex caseload of 30 -35 service users.
Key Purpose of Post |
Contribute to the provision of Social Care Services in the Community and other settings.
Provide an effective social work service including assessment, support, planning and reviews.
Operate a social work service within Departmental Policy, practice and procedures
To provide a specialist mental health social work role and to act as a Care Co-ordinator within the multi-disciplinary Community Mental Health Teams integrated with Bradford District Care NHS Trust in accordance with our partnership agreement and operational policy.
