Job Details:

• Senior Social Worker required for the Hospital Team The Hospital team facilitate discharges using D2A from Kingston Hospital and all Out of Borough Hospitals.
• Applicant must have knowledge and experience of Hospital Discharge and good IT skills Knowledge of LAS/ IAS and Google an advantage.
• Currently working from home but will be moving to Hybrid model in the future.
