Job description

The purpose of the Young Person's Practitioner post is to support the delivery of safe, effective, and creative services to young people where exploitation is a factor or a risk and where family breakdown is an immediate
risk. The role will include provide direct work to young people and their family network. The aims of the interventions is to disrupt exploitation and exploitation risks through joint work with the police and education providers
and to build stronger and more effective family relationship to enable families to lives together successfully. The post will contribute to achieving to positive outcomes such as young people feeling safer in the community,
supporting young people to access education, skills development and learning, supporting young people to access positive diversionary activities, develop resilience with families and friends' networks by enabling more
effective communication and addressing needs and risks with the adults which impacts on young people, creating hope and aspiration for young people to live a safe, happy and successful futures, reducing harm caused by
serious Youth Violence and reducing offending and repeat offending by young people.

The role will be a combination of case holding work and secondary allocations working alongside a social worker. Both aspects of work will have oversight from a Practice Manager who is a qualified social work. The case holding work will be with children in need. The secondary allocation work will involve work with the following case statuses CIN, CIC and ROTH /CP.
