Our client, Wirral Community NHS Trust is looking for a Social Worker to join their Adults team.

Part of Access and Intermediate Care Division, the role will be to complete 6 weekly reviews to support patients being discharged from hospital

You will need to be:

Social work england registered
Eligible to work in the UK
Atleast 1 year post qualifying experience
