Our client is looking for a social worker or senior social worker to join their 0-25 Disability Service and play a central role in delivering high-quality and responsive social work across children's and adults' services (up to 25).
It's a role to assess, plan and review disabled young people for short breaks, wellbeing and safeguarding purposes under the Care Act. You'll act as the lead professional in statutory cases - including Transition (Care Act 2014) assessments and support planning, mental capacity work and safeguarding adults - while undertaking the full range of social work tasks with disabled young people and their families.
