Job description

The role is for a Care Coordinator Social Worker to work in the Trafford Older Adults Community Mental Health Team (CMHT) is a Secondary Mental Health Multi-Disciplinary Team (under GMMH Trust) working with Older Adults with complex mental health needs, with either a functional or dementia diagnosis. Older Adults CMHT based at Crossgate House in Sale.

Care Coordinator duties would include Care Coordination/Case Management of Service Users under Care Programme Approach (CPA), managing a caseload (including care planning, risk assessment and management, crisis planning), social care commissioning, team duty work - carrying out initial assessments.
Applicant would need to demonstrate approximately 2 years post-qualifying experience and working knowledge of The Care Act, Mental Capacity Act, Mental Health Act , CPA, Safeguarding Adults.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
