Job description

Sutton's Hospital Pathway Team in Adult Social Care supports Sutton residents through the whole process surrounding hospital admission, enabling individuals to regain independence. As part of the Sutton Health and Care alliance, Adult Social Care have strong multi-disciplinary partnership working across Health and Social Care, and our in house reablement team which is part of this alliance. The Hospital Pathway Team is a multidisciplinary team which works with individuals to prevent hospital admission, to support hospital discharge, and throughout the whole period of reablement following discharge from hospital.

Social workers within the team are required to complete short assessments in crisis to ensure appropriate support is provided in a timely manner to both prevent hospital admission, and in order to facilitate hospital discharge. Social Workers then work closely with Sutton's in house reablement team (START) to provide holistic and strength based eligibility assessments in conjunction with the multidisciplinary team comprising of support workers, physiotherapy, occupational therapy, social workers and assessment officers, to identify ongoing needs following a period of reablement, including those of carers. As part of this role, Social Workers will also be required to complete Mental Capacity Assessments and Best Interest Decisions, contribute to NHS Continuing Healthcare assessments, support individuals to address issues relating to the habitability of their property, and make applications to Court of Protection regarding deputyship for finances.
