Senior Social Worker - East Riding of Yorkshire Council - Hospital Social Care Team - Up to £40ph!

East riding council have the exciting opportunity to invite short term agency staff initially for 3month to join our expanding hospital social care team. The service is going through a redesign implementing a SPA, Home first model with a creative balanced of a bed based discharge to assess service.

This opportunity developed from investment in the service where the staffing has been robustly increased to meet service demand. Mainly the posts will cover 37hours Monday to Friday and bespoke working agreements can be discussed with the preferred candidates. The service is 7 days a week and as the successful candidate settles into the role we can offer hours over weekends as demands require.

The role is a blended approach office based during a targeted induction phase and then a mixture of home working and office based. To meet the demands of the service we are required at times to attend wards and care settings to complete essential visits and all appropriate PPE and precautions are provided. As part of the COVID response we are currently supporting best practice of two lateral flow COVID 19 tests weekly.

The roles are based on two sites Castle hill hospital HU16 5JQ (main office) and Hull Royal infirmary. During the working day there may be a need to transport between site where there is a local bus service or mileage will be paid in line with the councils agreement.

The role is completing proportionate care acts assessments, Care plans, mental capacity assessments and agreeing timely discharge plans. Communication is essential skills not only with clients but maintaining updates for the cases as they develop. We currently use AZEUS to record a prior knowledge would be beneficial. The pace of work within the service means that organisational skills and the ability to self manage your case load is essential. Despite the need to discharge timely good strengths based approach and client centred assessment is essential throughout all assessments and recordings.
