Our client, Reading Council is looking for a Social Worker to join their discharge to assess project.

This is an exciting opportunity to deliver on the new pathway which will empower service users to live within the community with minimal home care support and to work as part of a passionate team whose ethos is to work in an efficient and effective way to enhance individuals well-being.

This is a fixed term project for 20 weeks.

The work will be 70% remote and 30% onsite - you must be able to commute into Reading to do visits.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
