An exciting opportunity has arisen for a Preparing for Adulthood Practitioner/ Social Worker or Occupational Therapist to join Reading Council.

Our Client is looking for an experience individual:

To support young adults entering adulthood and promote independence
To support carers and young people's families.
To provide the highest standard of care and commitment to supporting young people with disabilities in ensuring that as a transition service we are contributing to the outcomes set in 'The Care Act', 'The Children & Families Act' and related Guidance.
To work effectively with partnership agencies including health and education to ensure that all agencies are working towards the same outcomes for young people and contribute to Education, Health and Care plans as appropriate.

You will need to be:

HCPC registered
Eligible to work in the UK
