The role of the Reablement Social Worker is to liaise with therapies to see how the patient is progressing toward their outlined goals. As the client progresses with support a decision will be made as to whether they will require long-term support. The Reablement Social Worker must complete Care Act Assessment and implement formal care before the end date of Reablement.

The lead therapists within the team are OTs Physiotherapists and Nurses who co-ordinate the package and link in with care staff to ensure that the individual is making progress. Reablement POC run up to 6 weeks but can be shorter therefore it is important that we begin our work with the client as soon as possible to avoid delay.
