The Westminster Community Mental Health Teams are seeking to recruit committed, highly motivated Mental Health Social Worker/AMHP to join the team. Westminster Rehab team is tasked with care co-coordinating a cohort of clients in high and medium support accommodation across the borough of Westminster. The team also monitor and review 24 hour placements out of borough for Westminster spot purchased placements. The teams focus is supporting clients towards greater independence, building their confidence and enabling them to step down to less supported accommodation ultimately. This is very much done in partnership with staff from the supported accommodation. Placements are continually monitored ensuring an appropriate care package is in place whilst focusing on ensuring an active step down plan is in place. Team members work with commissioners ensuring that out of borough placements are delivering high quality care within cost efficient parameters.
