Remedy Recruitment Group are currently seeking an experienced Qualified Social Worker to work within a Hospital Team for Ealing Council.

Job Purpose

An opportunity has arisen for a social work post in the Hospital Assessment Team.

We are looking for candidates with the energy and enthusiasm to embrace this challenge and support the management group in the development of the Independent Living Service. You will have extensive experience and a strong commitment to high standards of professional practice. As a bastion of good practice, you will expected to demonstrate expertise in problem solving in respect to cases. Additionally you will have experience in maintaining robust relationships with the integration of health and social care , working housing and third sector professionals.

You will have experience in the following:

- Managing a caseload
- Working in a fast pace hospital environment
- Attending Multi - disciplinary meetings
- Managing a ward
- Joint meetings with health
- Conducting a Mental Capacity Assessment
- Completing Care Act Assessments under the Care Act 2014

You must also be willing to work weekends

You will need to be:

HCPC registered

Eligible to work in the UK
