Qualified Social Worker, Hospital Discharge, Portsmouth, up to £30-£32

If you are an experienced Qualified Social Worker with a background with Hospital Discharge, we want to hear from you.

Essential requirements:

• Degree in Social Work or equivalent qualification
• Registration with Social Work England
• ASYE complete
• Substantial experience within a Hospital Discharge Team
