Job Description:

The Hospital Assessment and Community Review team in Havering is looking for enthusiastic and dynamic Social Workers to come and join our team. The role will primarily support the acute hospitals and also our linked community hospitals across Havering. We operate under a trusted assessor and discharge to assess model, with social workers supporting the very complex hospital discharges. Follow up assessments under the Care Act 2014, are also a part of the teams remit, to establish long term needs post discharge. Our service runs from 9am-5pm Monday to Friday and 9am-5pm on Saturday with the potential for some Sunday working.
