Qualified Social Worker - East Riding of Yorkshire Council - D2A Surge Team - Up to £35ph!

The role of the Social Worker is to be able to work as an autonomous professional, demonstrating expert and effective practice in complex situations. This role will be supporting the newly formed D2A Surge Team for adults across the East Riding of Yorkshire. During the Covid-19 pandemic, existing hospital discharge processes were rapidly reviewed to expedite hospital discharges and maintain flow. National guidance was clear that social care needs assessments should take place in a community setting and not during the hospital inpatient stay. East Riding of Yorkshire Council adopted new processes to implement this guidance including commissioning the Home First Support Service to support the Discharge to Assess (P1) process across a number of localities.

In order to ensure sufficient flow across the pathway and to enable those requiring long-term care following the D2A process / a period of reablement, it is important that subsequent assessments can take place in a timely way. Increased demand in the Community Wellbeing Teams has resulted in a number of stacked assessments and hence the Surge Team is being formed. This team will: complete all the stacked D2A Care Act assessments in the Community Wellbeing Teams; complete all outstanding assessments for people currently in receipt of the Home First Support Service (currently operational in Haltemprice, Holderness and Beverley localities); and complete all the new D2A Care Act assessments from the HUTH system (Hull University Teaching Hospital Trust) for the duration of the contract.

We require experienced Social Workers in Adult Social Care to undertake dynamic Care Act care and support assessments, support plans and risk assessments. The Social Worker should have working knowledge and experience of completing Mental Capacity assessments and the Best Interest Process.
