Job description

Our client, Knowsley Borough Council is looking for a Qualified Social Worker to join their Transitions team

The transition team works with young people from 14-25, the aim of work with 14-17 year olds is to plan for care act eligibility assessment (social care need assessment), ensure young person, family and other involved in their care are prepared for transition to adult services, that they have care planned and in place ahead of their 18th birthday and that following this their support is reviewed and support provided to ensure further smooth transition from education to community based placements and to settle young people into long term sustainable and appropriate accommodation (including sustaining support within family home).

The post holder will engage in social care needs assessments of young people, develop support packages and review these, utilising mental capacity act assessments where appropriate. They will initiate cop dol processes if required, work closely with colleagues in children's social work teams, leaving care teams and adult services to ensure planning is in place for a smooth transition to adult services. They will work closely with health colleagues in range of teams including Learning disabilities, mental health (children's and adults), and ICB.

The post holder will be accountable to the senior practitioner and service manager for the transition team and will be expected to work closely with team members
