Job description

Our client, Oxfordshire County Council, is looking for a Adult social worker to join their discharge to assess team.

Timely Care Act Assessments and strength based support planning to support flow through our Discharge to Assess beds to facilitate flow throughout the hospital system.
Clients are transferred out of the Hospital to a Discharge to Assess bed and we need you to complete the Care Act Assessments and support planning for the identified eligible longer term needs. Mental Capacity assessments and Best Interests decision will be integral to your support planning. Experience in identifying safeguarding issues and following protocols and processes is essential.
The roles are agile and remote working is anticipated for most of the cases. Some face to face assessments will be required. As the posts are agile there will be some flexibility in the hours which can be discussed at interview. The expected maximum length of stay in the D2A beds is 28 days.

All locums will be attached to one of the permanent hospital social work teams for supervision and support.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
