Remedy Recruitment Group are currently seeking an experienced Occupational Therapist to work for Islington Councils Discharge to Access Team.

Job Purpose

• Working collaboratively with therapists and carers to support and facilitate the assessment and rehabilitation of patients post hospital discharge in the community.
• Completing welfare calls and liaising with care agencies to ensure patients are receiving support required post hospital discharge.
• Completing follow up visits to assess equipment prescription and correct use.
• Completing referrals onto relevant community teams as indicated

You will need to be:

• HCPC registered
• Eligible to work in the UK
