Purpose of the Post:

• To be responsible for assessing the functional needs of adults in their homes and in a community setting, in line with all relevant legislation e.g. the Chronically Sick & Disabled Persons Act 1970, the NHS & Community Care Act 1990, and the NHS Plan 2000, and in accordance with the Service's, the Council's and the Primary Care Trust's policies and procedures.
• To make recommendations and implement a wide range of simple cost effective interventions to meet individuals' assessed need in accordance with an agreed care plan, to optimise client function and quality of life.
• To be an actively participating member of the specialist ACOT team, contributing positively to the overall function of the department, supporting the role of Occupational Therapists.
• To ensure that intervention is responsive, and sensitive to the social, cultural, linguistic and religious needs of clients in accordance with equal opportunity policies
