Job description

Our client, Nottinghamshire CC is looking for an Adult social workers to join their Discharge to Assess team.

We currently have 3 vacant social worker posts within our Discharge to Assess South Team.
1 of the positions is to support and assess people that are in the acute hospital (Queen's medical centre and Nottingham City Hospital)
2 of the posts are required to assess people that have been discharged from the acute setting. This is predominately community facing and people will be assessed at home or in community rehabilitation/bedded facilities.
We are currently working to a hybrid model. The expectation is people will work from home and come into the office space once/twice a week. We are assessing people remotely however if the person requires a face to face assessment we are completing these. We will obviously risk assess the situation and ensure all the COVID-19 guidance is followed and PPE supplied.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
