Our client is looking for a Social kWorker who must have worked in a transition team previously for a minimum of 2 years.

The role will require you to work on extremely complex cases - working with residents that have medical, mental health and LDD conditions (often several).

Experience of LAC and Leaving Care in addition to transition would also be an advantage.

This role is offered on an ongoing basis to start ASAP.
