Remedy Recruitment Group are currently seeking an experienced Mental Health Social Worker to work within Lambeth Council.

Job Purpose

This post is a social worker post within the Community Living and Support Service (CLASS). This team is part of a pilot that has 2 social workers, a manager, and support workers to review patients on acute psychiatric wards to assess need and where appropriate draw up care and support plans to facilitate a discharge. The social worker will be expected to use commissioned services in the first instance and to signpost where appropriate. The social will also be expected to attend funding panel where appropriate. The will be little safeguarding work in this post. We are looking for someone who is self-motivated, a team player, confident in giving advice and delegating where appropriate, and willing to represent the social view particularly during the discharge process, amongst the MDT on the ward.

You will need to be:

HCPC registered
