Our client, Medway Council is looking for a Social Worker to join their Transitions & Review team.

• This role is within the Transition and Review Team - specifically reviewing clients with the aim of reducing the waiting list and supporting hospital discharge team
• Must be a car driver (hold a full licence and have a car)

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
