Our client, Manchester Council is looking for a Social Worker to join their Hospital Discharge team.

Experience of hospital discharge is preferred.

Understanding of Discharge to Assess model.

Ability to work flexibly on a rota basis Monday - Sunday between 8 am - 8pm (35 hour week).

Experience of Liquid Logic preferred.

You will need to be:

Social work england registered
Eligible to work in the UK
Atleast 1 year post qualifying experience
