Qualified Social Worker - Lancashire - Children with Disabilities Team

The Children with Disabilities team is a specialist social work service, working with children and young people with severe and profound disabilities and their families. This includes children with complex health needs and behaviour that challenges. We undertake the full range of social work roles and task for children and young people who meet our criteria, including assessment work, supporting children and young people through CiN Plans, CP Plans and when they need to become children in our care. We support our care leavers and support young people and families to transition to adult services. We work in very close partnership with other services and professionals to support our families. We care about our children, young people and their families and embrace a strengths-based, partnership approach to providing support and relationship-based social work We need one agency social worker, preferably with experience in working with severe and profound disabilities to work in the team covering Burnley and Pendle (would may also cover other areas of Lancashire in the East and potentially outside of this. The initial contract would be for three months whilst we recruit into vacancies. The CwD Service offers a wide range of experiences and social work practices and the opportunity to make a real difference through supporting children and young people with disabilities and their families. We strive towards workers having a more manageable caseload, offering the best training and support and increasing the range of services we offer to ensure our Social Workers make a positive impact on a daily basis. In joining the service you would be welcomed as part of the team and have the support from experienced colleagues in the other East-based team and across the wider services. Current the office base for the Burnley and Pendle CwD Team is being identified and therefore there is increased agile working, utilising a range of office bases and the local short break children's home for Team working. The post you are applying for is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (as amended in 2013). If successful you will be required to apply to the Disclosure and Barring Service (DBS) for a 'disclosure'. You must be registered (Social Work England) to be appointed as an agency Social Worker. You will be required to provide a car for use in connection with the duties of this post and must be insured for business use. However, we may consider you if you cannot drive because of a disability.

Your main responsibilities include:

* Hold and effectively manage a complex and varied caseload with appropriate supervision, guidance, and support, reflecting the function of the team and the Vulnerable People Service in accordance with policy, procedures, guidance and legislation.
* Undertake work within legislative frameworks including the preparation of written statements and representations to the judiciary at Court hearings.
* You will be accountable for complex decision making in relation to caseloads ensuring professional judgement, service user involvement, needs led assessment, critical reflection, and analysis to inform this.
* You will identify and assess levels of risk and need (within statutory frameworks) in often complex situations. Undertake safeguarding investigations and deliver protective and/or supportive services for individuals at risk and maintain up to date assessments, care plans and reviews.
* You will be preparing statements of need/care and support plans, in consultation with other agencies as appropriate, that reflect the wishes of the individual and lead to the commissioning of personalised packages of care.
* You will undertake direct work with adults and their families in line with care plans.
* You will work co-operatively with both internal and external colleagues across multi - agency boundaries.

Essential requirements:

* Social Work England registration
* Eligible to work in the UK
* 2 years+ post-qualified experience
* Degree level or equivalent in Social Work
