Job description

Our client Isle Of Wight Council is looking for a Social Worker to join their Adult Social Care team.

We are looking for a Qualified Social Worker to work as part of Adult Social Care at St Mary's Hospital, Newport, Isle of Wight. You will be undertaking full care act assessments in line with current legislation to support hospital discharges within the Isle of Wight community, determining peoples long term needs and supporting carers. Working as part of an integrated discharge team in a hospital setting with occasional requirement to support the hospital duty team with trusted assessment work.

Worker needs to be based on the Isle of Wight for this post all week.

You will be required to work 5 days per week Monday - Thursday, 8.30am until 5pm and Friday 8.30am until 4.30pm = 37 hrs Occasional weekend working will be required to be taken back as time in lieu.

The ideal candidate will have a good understanding of the role and the complexity of the work undertaken to achieve the best outcomes. The key role will be to facilitate and support individuals to complete a needs assessment using a strengths-based approach, which focuses on the skills and abilities, considers their networks of support both within families and the wider community. Your work will be delivering on the Isle of Wight's 'Care Close To Home' strategy, which enables individuals to live fulfilling independent lives in their own home.

Key experience required: - Clear experience and operational knowledge of working in a busy and pressurised front-line team - Excellent communication skills (written and verbal) one to one and with client groups - Complex case management; working with children and families as well as adults - Good collaborative skills, maintaining strong relationships with partners - Safeguarding and continued Healthcare - Sound understanding of current Social Care legislation including Care Act, Mental Capacity Act, Mental Health Act etc - Excellent organisational and prioritisation skills - Able to work independently to strict deadlines under pressure - To demonstrate person centred working - Knowledge of financial processes (benefit entitlement) - Ability to utilise ICT at a high level and adapt to new ICT systems We would ideally like to start someone in post on Monday the 18th October, however for the right applicant we could wait a little longer to allow for notice periods.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
