Hospital based social work team that supports the discharge to assess process. The team tracks local residents from admission, through the discharge process and then reviews the care within six weeks of discharge before handing the case over to the community social care teams. Team completed Human Rights Assessments as well as Safeguarding enquiries. The team are involved in complex discharges, supporting the hub with capacity assessments and best interest decisions. The team has a daily duty worker who is based within the hub and who facilitates and urgent action required to support discharges for the day.
