Job description

Duties will be to;

  • Work in a newly developed Children's Services Placement Team serving H&F's frontline social work teams to ensure all H&F looked after children are placed in secure, stable and safe placement arrangements so that they achieve the best outcomes.
  • Organising and facilitating placements for looked after children, children at risk of becoming looked after and care leavers aged 0 - 25 across a range of placement settings including, but not limited to, foster care, residential children's homes, residential schools and semi-Independent living units.
  • To lead the placement finding, matching and contracting process to support social work teams to make and maintain stable and cost-effective placements, which meet the assessed needs of looked after children.
  • Ensuring robust quality assurance arrangements are in place across all placement categories and ensuring value for money.
  • Completing all relevant documentation for each placement such Individual Placement Agreements, Child Profiles, Needs and Risk Assessments.
  • Ensuring we achieve the best placements at the right price and that there is clear breakdown of placement costs.
  • Knowledge of using the Mosaic social work casework system and the Commissioning Alliance's Careplace system for all vacancy searches.
