1. Experience in working in adult learning disabilities, older adults or transitions,
2. Identify needs of clients, support and intervention of the care act.
3. Experience and strong knowledge of Continuing health care
4. Experience in adult safeguarding
5. Manage staff complexity, dynamics within a team.

This role will predominantly be home based and will be until at least end of March 2022.
