An East London borough are currently offering an exciting opportunity for a registered Nurse who is experienced in complex case management, has a good understanding of the National framework for NHS Continuing Healthcare & NHS Funded Nursing Care and is keen to develop further in this field. Part time and flexible working will be considered.

The CHC Team are responsible for undertaking clinical assessments, quality reviews and commissioning services and support for individuals subject to various NHS eligibility criteria, such as The National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care, The Mental Health Act, Transforming Care and Neurological Rehabilitation pathways.

You will need to be able to evidence your knowledge of NHS Continuing Healthcare pathways and tools, have comprehensive communication, assessment and care planning skills, which will include complex case management skills.

You will also need to demonstrate the ability to problem solve, multitask and manage junior staff effectively.
