Our client, Brent council is looking for a social worker to join their urgent care team.

• To carry out needs led assessments of adult within the guidelines of Community Care Act.
• To care manage straight forward and complex cases including Safeguarding Adults and Deprivation of Liberty.
• Co-ordinating and reviewing multi-service care packages.
• To support the team in meeting individual and team performance targets. To actively contributes to its continuous improvement.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
