Social worker in Hospital discharge Team , assessment of patients in the community under the discharge to Assess pathway. Based at QEH, but will cover all hospital discharges. Please note - under current government guidelines, all assessments are to be carried out in the community. As such, candidates will be expected to go into care homes or people's own homes to assess.

Essential requirements:
* Degree in Social Work or equivalent qualification
* Registration with Social Work England
* ASYE completed
