Job Description:

A strong working knowledge and awareness of the Care Act, Continuing Health Care, Mental Capacity Act, Safeguarding, Delayed Transfers of Care and CHC . Completing daily auditing of assessments, ensuring their compliance with the Care Act 2014, that they reflect the needs of those being assessed and ensure they adhere to the agenda of personalisation. Ensure care and support are delivered in a cost effective manner, identifying networks, local resources, assistive technology, equipment and adaptations to support this. Provide case management oversight and supervision. To be a source of expert advice for health and social care staff, students and partner staff in order to maintain the highest level of professional service. Must have knowledge/experience of performance and absence management. Demonstrate experience in developing good working relationships and partnership working with other departments and services within London Borough of Hackney, City & Hackney CCG, Homerton Hospital, and Integrated Independence Team. The post holder will be expected to work with an in-depth knowledge of both legal and statutory frameworks relating to social work practice. Demonstrate an understanding of the performance indicators that are relevant to the work of the team and work within the required time frames. To have a detailed knowledge of the services for carers/service users provided within Hackney and how to access these. Excellent written and verbal communication skills, including case recording using Mosaic and IT systems. 1 year of management experience is desirable.
